Ketamine Cut Suicidal Ideation in 48 Hours. The Placebo Group Started Slipping at Day 10

Ketamine Cut Suicidal Ideation in 48 Hours. The Placebo Group Started Slipping at Day 10

A commentary published today in the Journal of Clinical Psychiatry makes the case for ketamine plus buprenorphine as a bridge out of a suicidal crisis. The commentary itself is not new data. The trial underneath it is, and it lands on a problem every ketamine clinic already knows about.

The clinical calls are yours. The cost of running the building does not have to be. HealingMaps’ GPO, the first built for the ketamine industry, gets member clinics 40 percent or more off medical supplies, 15 to 20 percent off malpractice and liability premiums, discounted LegitScript certification, and 20 to 30 percent off HIPAA compliant payment processing. Key Takeaway Detail
The trial 50 adults infused at a single Stanford outpatient center, 45 analyzed
The design Open-label ketamine first, then four weeks of blinded buprenorphine or placebo
The doses Ketamine 0.5 mg/kg over 40 minutes, buprenorphine 0.2 to 0.8 mg a day
The result Suicidal ideation scores fell 11.6 points on buprenorphine against 6.3 on placebo
The limit Depression scores did not differ between the two groups
The debate Four letters ran in the journal today, alongside the authors’ reply

What the Stanford Team Set Up

That ketamine reduces suicidal ideation quickly is well documented. How long the effect lasts is the open question.

The Stanford group ran a randomized, double-blind, placebo-controlled trial at one outpatient center.

Every participant received a single open-label ketamine infusion, 0.5 mg/kg over 40 minutes.

Forty-eight hours later they were randomized to sublingual buprenorphine, 0.2 to 0.8 mg a day, or a matched placebo.

That ran four weeks. The measure was the Scale for Suicide Ideation, scored weekly through day 31.

Enrollment took from November 2020 to March 2025. Fifty patients were infused and 45 completed at least one week.

Where the Placebo Group Started to Slip

Both arms improved sharply in the first 48 hours. That part is not contested.

The separation came later. Scores in the placebo arm began climbing back around the 10-day mark.

The buprenorphine arm held its gain through day 31.

Mean change on the ideation scale was 11.6 points on buprenorphine with 23 patients. Placebo moved 6.3 points with 22.

The time-by-treatment interaction was significant at p below 0.001.

The rationale is a receptor argument we have covered before, in a piece on ketamine acting on more than one brain system.

Read the Effect Size Before the Headline

The effect size was a Glass delta of 0.76, with a confidence interval of 0.11 to 1.39.

The lower bound sits just above zero. With 45 patients at a single site, that is about what you would expect.

Depression scores did not separate. Only the suicidal ideation measure did.

That is a narrower claim than it first reads as, and it is worth stating narrowly.

No serious treatment-related adverse events were reported.

Four Letters Ran the Same Day

The American Journal of Psychiatry published four letters about this trial on October 19. The authors’ reply ran with them.

One is titled simply, ketamine extension or buprenorphine maintenance.

That question has history behind it. A multisite trial in the same journal in 2016 found that ultra-low-dose buprenorphine on its own reduced severe suicidal ideation against placebo.

So the drug has a track record here without any ketamine attached to it. Separating the two effects is the harder task.

A second raises durability, specificity and confounding. A third asks how the antidepressant and antisuicidal effects relate.

The fourth comes from emergency physicians who want the approach tested in their own setting.

That same group wrote the commentary that prompted this article. Their interest is the emergency department, not the infusion suite.

A finding that draws four letters in one day is a live argument. It is not a settled result.

Why This Is Not a Protocol Change

Buprenorphine is an opioid. In this field that alone is reason for care.

It carries no approval for suicidal ideation, so any use here would be off label.

The trial also excluded patients with substance use disorder, as the 2016 trial did before it.

The commentary notes that up to 42 percent of emergency patients presenting with suicidal ideation carry an opioid use disorder diagnosis.

The population most likely to raise a question is the population the trial left out.

Off-label decisions carry a documentation burden of their own. We set that out in 8 gaps that create malpractice exposure.

The Part a Clinic Can Use Now

Take the second drug out of it and a scheduling fact remains.

Left alone, the benefit of one infusion began fading around day 10 in this trial.

A patient seen for suicidal ideation and booked a month out is uncovered through exactly that window.

Whatever the bridge turns out to be, the calendar is the piece a clinic controls today.

For patients on esketamine there is already an approved path here, which we covered in our look at the acute suicidality indication.

A Question About the Handoff

The trial does not ask whether ketamine reduces suicidal ideation. It takes that as the starting point.

What it tests is what happens in the weeks after, and whether anything holds the gain in place.

One small trial does not answer that. It does put a number on how quickly the window closes.

This article is for informational purposes and is not medical advice. Buprenorphine is not approved by the FDA for suicidal ideation. Anyone in crisis can call or text 988.
