Getting Started
Your physician, or whoever is treating you, must fill out Life Infusions’s Referral Form. The clinic will also request your review and signature of other forms, such as the Notice of Privacy Practices and HIPAA authorization form, in addition to your medical records from another doctor, a therapist, lab, or hospital to better understand your health journey so far.
Introductory Phone Call
After receiving the Referral Form, LifeInfusions will give you a call to discuss the treatment, schedule your infusion, and answer any questions you may have.
First Visit
You’ll take part in a short Welcome Meeting to confirm that you’re in good health and a good candidate for ketamine infusions.
During and after the actual infusions, you’ll be in a comfortable room with access to the following amenities:
