The At-Home Competitors Are in the Same Auction

That is a direct substitution threat, not an adjacent one. A patient searching for ketamine treatment who clicks a national telehealth ad has been removed from the local market entirely.

Two of the Advertisers Have Nothing to Sell Yet

The map surface is the other place. It sits above the paid block on local searches and is not purchasable the same way. That makes Google Maps visibility worth more than a bigger keyword budget.
What to Do This Quarter
Check your own cost per click on category terms against last year. If it has climbed, this is why, and outbidding a pharmaceutical company is not a strategy.
Shift budget toward local intent and toward the terms only you can answer. Audit which national brands appear on your own clinic name, since brand-term poaching is common and cheap to defend against.
Then treat organic and map presence as the durable asset. Auction prices can double. A well-built local listing does not reprice.
