The questionnaire generates a 22-page report which Dr. Costello reviews prior to the first appointment with each new patient. This questionnaire may be completed at home or here in the clinic’s office, for your convenience. If the patient is 18 years or older, they will complete the questionnaire themselves. If the patient is younger than 18, the parent or legal guardian will be responsible for completing the questionnaire on the patient’s behalf.
